|appearance=Sanmoto Gorōzaemon wear a blue Edo Period samurai dress, red uwa-obi, and tattered hakama which only cover her right side, red sandals, wear wrapped cloth on chest area, white fundoshi, and has demonic shadow with multiple eyes behind her. Sanmoto has olive green chonmage hair with one huge yellow demonic eye, demonic ears, and demonic right hand which carry a wooden mallet that he gave to Heitaro.
